CHRONOBIOLOGICAL PERSPECTIVES ON DIET, CARDIOVASCULAR RISK, ENDOCRINE CANCER, AND PREMETABOLIC SYNDROME.

The words of Rene Dubos, “Think globally, act locally” (UN Stockholm Conference (1972)), and his ‘5’ Es, ecosystems, economics, energy and esthetics, do, in a sense, apply to the health of man. Halberg and co-workers at the University of Minnesota have recognized the need for clinicians and others concerned with the proper evaluation of a patients rhythmic blood pressure by "First do no harm" by not flying blind (Halberg et al. Biologist 2009; 56: 209-214). Towards this goal, standards have been set in study design based on the collection of adequate ambulatory blood pressure (BPM) monitoring information, recognition and quantification of Vascular Variability Disorders (VVD; e.g. CHAT (Circadian HyperAmplitude Tension), and the compilation of reference standards for selected demographic variables. Thus after 2-3 decades of intensive research tools are now available for screening populations for VVDs, and for optimizing the treatment (e.g. circadian time and dose) following myocardial infarction and stroke. In Durham and York Universities (Murphy, J., Gathercole, J., Fuat, A., Conway, B., Wilson, D.W. Littlewood, E. (http://www.tsimtsoum.net/tsimtsoum_krakow_workshop_programme.pdf)) information was obtained on “Working memory deficits in untreated and treated hypertension” whereupon significant deficits in verbal working memory were found in the hypertensive group relative to controls both before and after antihypertensive treatment. A strong and statistically significant inverse correlation was observed between baseline systolic pressure and verbal STM. It is possible that changes in STM in the elderly may be a signal for risk of stroke wherein ABPM would be invaluable for diagnosis and treatment.

In relation to endocrine cancer such as breast and prostate, Griffith’s group at Cardiff University, recognized that hormones exhibited rhythmic structures, that were potentially important in human development (e.g. progesterone in pre-and post-menarcheal girls in the UK Thailand pre- and post-pubertal boys, and in adult male exercise) and cancer risk (breast and prostate) and that their ‘rhythmic’ finger print could be obtained, not automatically as in ABPM but in saliva samples that could be collected frequently. Breast cancer risk is low in Thailand and interest was focussed on phyto-oestrogens, such as tuberous roots of the Kwao Keur and its Burmese preparation Pauske. The consumption of isoflavonoids, lignans tend to direct attention towards a ‘protective’ effect of dietary vegetables, fruits and plant products and suggest that the risk for Western diseases relates more closely to inadequate intake of plant products, suggesting a ‘natural’ Tamoxifen and such oestrogen products are found in expressed prostatic fluid.

Singh, De Meester and co-workers have also been stimulated by Eastern medicine and dietary consumption of fruits and cereals and by their scholarly studies of ancient medicine in India (“Foods which are bitter, acid, salted, burnt, fried, and pungent, give rise to pain, mental stress, and diseases” (3100BC); Charaka (600BC). “Heart attack is born by the intake of fatty meals, overeating, excess of sleep, lack of exercise, and anxiety; Sushruta “Excess intake of fatty foods and lack of exercise causes obesity and narrowing of the channels taking blood to the heart. It is useful to use guggul, triphala and silajit in the treatment” and these herbs are known to have high content of antioxidant flavonoids, vitamins and minerals as well as fibres.

Low dietary monounsaturated fatty acids and n-3 fatty acids, are important in the pathogenesis of metabolic syndrome an activity pursued by Sanchez de la Pena in Mexican cohorts. Increased sympathetic activity with greater secretion of neurotransmitters in conjunction with underlying long chain polyunsaturated acid deficiency, and excess of proinflammatory nutrients, may damage the neurons via proinflammatory cytokines, in the ventromedial hypothalamus and insulin receptors in the brain. It is possible that their supplementation may be protective and this is an intervention study being conducted in Poland, called the Chronomics Project (De Meester and co-workers) of which most of this group is a part. Selected examples of these chronobiological perspectives will be presented at this conference but the Chronomics project will be discussed elsewhere.
